Intake forms
An intake form is the set of questions a client answers when they book with you: a vehicle make for a detail, a skill level for a lesson, allergies for a massage. You build yours in Settings → Booking, under Intake form.
You start with a ready-made form based on the vertical you picked during onboarding. Change as much or as little of it as you like.
Editing a form
- Reorder fields by dragging them.
- Rename a field by typing over its label.
- Turn Required on or off for each field.
- Remove a field with the trash icon.
- + Add preset field… adds a question from your vertical’s standard set.
- + Add custom field creates your own question (for example “Gate code”).
Nothing changes on your booking page until you click Save intake form.
Removing a field never deletes answers you’ve already collected.
Field types
| Type | Good for |
|---|---|
| Short text | A name, a vehicle model, a gate code |
| Long text | Goals, anything that needs a few sentences |
| Dropdown (one) | Picking one option from a list |
| Multi-select | Picking several options from a list |
| Checkbox | Yes/no questions |
| Date | A birthday, a due date |
| Phone | A second contact number |
| A second contact email |
For Dropdown and Multi-select, type your options one per line.
Asked every visit, or saved to the client
When you add a custom field, When to ask decides where the answer lives. Each field is tagged so you can tell them apart:
- Asked at booking (Ask every visit): the client answers it on your booking page each time they book, and the answer is saved with that appointment. Use it for things that change from visit to visit, like “What do you want to work on today?”
- Client profile (Save to client profile): not shown to clients. It’s a fact you keep about the client, like a skill level or a school. You fill it in yourself from the client’s page under Clients → Custom Fields.
- Post-visit (you): notes you record after the appointment. Clients never see these.
Client profile fields come from your default form.
More than one form
If you offer different kinds of work, you can keep a separate form for each. A coach who also does sports massage doesn’t need to ask massage clients about their batting stance.
- Click + New form.
- Give it a Form name, like “Massage intake”.
- Under Start from, pick Blank or any vertical’s field set. You aren’t limited to your own vertical, so a coaching business can start a massage form from the personal-care questions.
- Click Create, then edit and save it like any other form.
Use the menu at the top to switch between forms.
Your default form
One form is always the default. It’s used for any service that doesn’t have a form assigned. To change it, select another form and click Set as default.
The default form can’t be deleted. Any other form can be deleted with Delete.
Assigning a form to a service
Open the service in Services and choose it under Intake form. Use my default form is the standard setting.
When a client books, Slot’d shows the form assigned to that service. If you delete a form that a service was using, that service goes back to your default form.
Starting over
Reset to defaults puts the selected form back to the field set it was created from. It only takes effect when you click Save intake form, and it never deletes answers you’ve collected.
Plans
The Free plan includes one intake form. Adding more needs a paid plan. If you already had more than one form, you keep them all. See Billing & plans.
